Creditable Coverage Disclosure Notice - 2009

Each year Hewitt is required to mail Creditable Coverage Disclosure notices to Medicare-eligible Retirees and/or their dependents.  Unfortunately, the Company did not make arrangements to have Hewitt send our office an advance copy of the notice again this year.

That being said, the Company has now provided the C&T office with an electronic copy of the 2008 notice.  The disclosure notice advises participants that Prescription Drug Plan coverage, provided by Alcatel/Lucent, is "at least as good as the standard Medicare prescription drug coverage."  In addition, the disclosure notice explains that Participants will lose their Company-provided prescription drug coverage if they choose to enroll in a Medicare prescription drug plan.

Please note that the use of Keystone East as a Medical Provider on the attached notice is to serve as an example.  The Creditable Coverage Disclosure Notices Retirees receive will be personalized and Keystone East will be replaced by their own Medical Provider.
