To All CWA Locals Representing Avaya Employees:

I need your assistance. We need to reach all Avaya Employees who have elected to add a "Domestic Partner" to their Benefits, and have not submitted the required "Affidavit."  Reminder letters and phone calls have gone out to these members to no avail.  If this required document is not received prior to February 28th, 2007, the "Domestic Partner" will be removed from the Employee's Benefits with a retroactive date of January 1, 2007.  We have 11 Avaya Employees who are in jeopardy of losing benefits for their "Domestic Partner."  Please ensure this is communicated to all of your Avaya CWA Members. 

Q: I sent my Affidavit in already, should I follow up with Avaya Healthy Decisions to make sure I'm ok?

A: Yes, better safe now than sorry later.  Simply call the Avaya Health and Benefits Decision Center at 1-800-526-8056.  You should always follow up with the recipient when sending important documentation.  

Q: I misplaced my Affidavit, how do I get another one?

A: Avaya Employees can obtain an affidavit by logging onto www.avayahealthydecisions.com.  Click on "Reference Material & Forms" (listed on the left side of the screen) then scroll down to "Domestic Partner" and click on "Affidavit of Domestic Partnership."  You may download and print this document but it must be signed and sealed by a Notary Public and mailed to AVAYA HEALTH AND BENEFITS DECISION CENTER, P.O. BOX 34330, LOUISVILLE, KENTUCKY 40232-4330.
